Waltham Forest Integrated Community Team are looking to recruit a band 6 nurse to work in the Community Respiratory Team.

We are looking for a motivated, enthusiastic, experienced and conscientious registered nurse with a passion for delivering excellent levels of clinical assessment. This post is an exciting role, allowing for care to be given closer to home, at the right time and by the right clinician.

The post holder will achieve this by working with the respiratory team in assessing, planning, co-ordinating, delivering and evaluating acute and long term care for the respiratory patient within the trust and across organisational boundaries.

You will be joining a well-established team of supportive and clinically experienced respiratory nurses that aims to work in an integrated manner, and have the added support of working under the direction of a band 7 line manager, with access to training in line with your professional development needs.

You will be expected to deliver a high standard of care to patients within the speciality who may have highly complex needs and to take an active role in supported discharge, admission avoidance, nurse led clinics, diagnostic spirometry, home management and Pulmonary Rehabilitation.

We are seeking a candidate who enjoys working within a multi-professional team, promoting delivery of a safe, effective and efficient service across Waltham Forest who can demonstrate excellent communication skills.

The role supports an integrated approach to patient care, providing Respiratory Nursing assessment and interventions for patients with complex respiratory needs within the community setting whilst working as a valid multi-disciplinary team member.

The main responsibilities of the post:

To represent the service and provide information to service users as required i.e. GPs, Primary and secondary care professionals, voluntary organisations, patients and their carers.

To provide safe practice to all patients referred to the Community Respiratory Team and liaise with Primary and Secondary Care Practitioners regarding the medical management of patients with respiratory problems.

To provide specialist advice to colleagues working in other clinical areas and act as the patient’s advocate.

To gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.

To provide clinical support and partake in management duties when required within the Respiratory Team to ensure continuity of care.

To participate in co-ordination and delivery of training to staff, ensuring competence in carrying out respiratory care and contributing to their professional development

Work closely with the Respiratory Nursing and medical teams, community matrons, and the Trust Lead for the strategic development and operational management of services for respiratory conditions to ensure best clinical outcomes for respiratory patients.

This is full-time post 37.5 hours per week, Monday – Friday.
You will be a car owner / driver or have access to a car and a current UK driving license is essential.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.  As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement – London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £3,659 to a maximum of £4,664).
