This position is in the single point of referral (SPOR) rapid response service covering Brentwood, Billericay, Basildon and Wickford Localities.
The Single Point of Referral Team is a rapid response community team and also short term intervention care up to 14 days. The team is a 365 day a year team working from 7am to 10pm.
Candidates need to have experience of working in the community and be able to demonstrate at interview the skills and competencies required to work effectively and efficiently. Be able to identify the skills required for this post and are able to identify own training requirements to fulfil this post.
The main responsibilities of the post:
Being able to carry out rapid response assessments and management of patients in a community setting
Work as an autonomous practitioner, managing and prioritising own workload according to local guidelines and identified need, working in a variety of settings, exercising a level of judgement, discretion and decision making in clinical care
To work flexibly to meet the needs of people, their relatives and carers and the service.
Promote collaborative practice across organisations, intermediate care, primary health care teams and localities, preventing duplication and promoting seamless care in the best interests of people using the services.
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement – Essex
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,000 to a maximum of £1,733).
You must have appropriate UK professional registration.
